Suspect aortic injury when:
A
- First rib fracture
- Apical capping
- Mediastinal widened
- Increased paratracheal stripe
- blunted aortic knob
- Left mainstem bronchus depression

Parkland Formula
A
Fluid resuscitation in burns:
2-4mL/kg LR for each percent of BSA of 2nd or 3rd degree burn; 1/2 given in first 8h

Definitely intubate CO poisoning if:
A
COHg >20% and symptomatic or
COHg >30%

Burn wound infections: timing
A
<7d Gram Positive
>7d- GNR and fungi (PsA, candida, Aspergillus)

Tx burns
A
- ppx abx not effective, dont penetrate
- topical abx decrease invasive infection
- mafenide acetate (burns, AGMA) or silver sulfadiazine (neutropenia)

Tx burns overview
A
Full thickeness- not systemic abx, debride necrotic tissue, apply topical abx
Partial thickness- biosynthetic, silicone-coated, silver dressings, NOT abx
