Treatment of prostate cancer Flashcards
(17 cards)
GnRH agonists
Goserelin, Histrelin, leoprolide, triptorelin
GnRH agonist adverse effects
decreases bone density, increases serum lipids, weight gain, HTN, diabetes, increased risk of CVD; sexual dysfunction, gynecomastia, and loss of libido
surge in bone pain for a short period if there are bony metastases
Degarelix moa
GnRH antagonism, reducing FSH and LH secretion
degarelix adverse effects
QT prolongation and increased hepatic enzymes; hypoandrogenic - hot sweats, weight gain, fatigue, impotence
estramustine moa
binds EMBP on prostate cancer and delivers the alkylator which function as a microtubule inhibitor
estramustine adverse effects
estrogenic effects - gynecomastia, mastalgia, impotence,
edema, thromboembolims, MI, PE, and stroke, increased hepatic enzymes
androgen receptor blockers
bicalutamide, enzalutamide, flutamide, nilutamide
prostate > central AR block
bicalutamide
Prostate and central AR block
enzalutamide and nilutamide
prostate only AR block
flutamide
enzalutamide adverse effects
CNS effects and URTI
flutamide adverse effects
BBW: hepatotoxicity
blood dyscrasias
nilutamide adverse effects
BBW: respiratory problems
heart failure, hypertension, blood dyscrasia, increased transit time from light to dark
Sipuleucel-T moa
autologous cellular immunotherapy designed to stimulate T cell immunity against prostatic acid phosphatase
sipuleucel-T adverse effects
paresthesias, infusion reaction, citrate toxicity
abiraterone moa
inhibitor of 17-alpha hydroxyls (CYP17)
abiraterone adverse effects
increased hepatic enzymes, Category X, produces an increased mineralocorticoid state, leading to HTN, fluid retention, and hypokalemia
