RCT with very severe ARDS:
P:F <30 for 3 hours, or <80 for 6 hours or pH <7.25 and CO2 > 60 for 6 hrs.

Intervention: VV ECMO with heparin vs standard ARDS tx

Results: stopped early because ecmo was better and reduced mortality to 35% v 46% with p= 0.07
