Insertion: deltoid tuberosity of humerus

Innervation: axillary nerve
Supraspinatus
Origin: suprasionous fossa of scapula
Insertion: greater tubercle of humerus
Function: abducts the arm
Innervation: suprascapular nerve
Infraspinatus
Origin: infraspinous fossa of scapula
Insertion: greater tubercle of humerus
Function: laterally rotates the humerus
Innervation: suprascapular nerve
Teres minor
Origin: lateral border of dorsal scapula
Insertion: greater tubercle of humerus
Function: laterally rotates the humerus when arm is adducted
Innervation: axillary nerve
Subscapularis
Origin: subscapular fossa of scapula
Insertion: lesser tubercle of humerus
Function: medially rotates the humerus
Innervation: upper and lower subscapular nerves
Teres Major
Origin: lateral border of scapula
Insertion: intertubercular sulcus of humerus
Function: extends, adducts, medially rotates the humerus
Innervation: lower subscapular nerve

Origin: iliac crest; thoracic and lumbar vertebrae
Insertion: intertubercular sulcus of humerus
Function: extends, adducts, medially rotates the arm
