what is personalisation?
people receiving care, and having choice and control.
this shapes the care given to best for the individual.
what does personalisation do for an individual?
they will feel
empowered, independence, control, choice, meeting their needs.

what is a personal budget?
a personal budget is a, amount of money allotted to a person who is in care. the amount of money is determined by the Local authorities based on the severity of their needs
what are the 2 types of personal budget?
1- Managed account
2- Direct account
What is a Managed account?
this is an account that is controlled but the Local Authority if an individual, lacks the mental capacity to handle the money.
What is a Direct Account?
this an account that the individual or a nominated person handles.
What is co-production?
this is when the individual, family, friends and carers make decisions as a collaborative, giving an individual control on decisions
what does EHCP stand for?
Education, health and Care Plan.
What does the EHCP do?
This looks at the Education and health concerning a child, and build a care plan around their needs.
What is a ‘All about me’ sheet?
this is a sheet that children fill out, which explains about their, name, religion, faith, but also about things they like and things they struggle with.
what is Person Centred care?
this is when an individual, who is receiving care, is put in the middle of every decision, meaning that any choices best suit the needs and wants of the individual.
what is Choice of residents?
this is the choice of where and how people live.
What are Adaptations?
When things are changed to best suit an individual, this can be done in and individuals own home.
What types of adaptations can be made?
Lower sinks and hobs
wider doorways
hoists to access baths and toilets
What is Meeting housing needs?
this is when an individual may want to stay in their own homes, which means that they need to make appropriate adaptations, which the social services are able to help with.
What are Geographical barriers?
when a person moves home their may be difficulties registering to a new General practitioners, or Local authority.
