Commonest micro-organism causing UTI?
E. coli
Microbiological classification of E. coli?
Gram negative, facultatively anaerobic rod-shaped coliform
Which organisms are seen in patients with underlying urinary pathology, immunosuppression or catheterised? (4)
Klebsiella
Proteus vulgaris
Candida
Pseumonas
Important risk factors for UTI particularly in older people (6)
Older age Female gender Structural abnormality Incomplete bladder emptying Indwelling catheter Diabetes
Why is routine screening for UTI not recommended in older people?
High proportion of older people have asymptomatic bacteruria; unnecessary antibiotics may cause more harm than good
What is the exception to non-treatment of asymptomatic bacteruria?
Patients undergoing invasive urological procedures
Features which increase the likelihood of UTI? What caveat is there in older people?
Dysuria
Urinary frequency
Haematuria
These may not be features in older people
How may UTI present in older people?
Dizziness (+/- fall)
Confusion
Nausea
What should be taken into account when advising older people about using cranberry products|?
Cranberry products can potentiate warfarin
Common condition in older men which predisposes to pyelonephritis?
Prostatic hypertrophy leading to retention
Presentation of pyelonephritis?
Fever
Flank pain
Costovertebral tenderness
Asymptomatic- diagnosed when found to be in renal failure
Contraindication to nitrofurantoin?
eGFR less than 60
Why should co-amoxiclav not be prescribed to older people?
Risk of C diff, particularly in hospital
Empirical treatment of uncomplicated UTI in female
Nitrofurantoin 50mg qds or 100mg MR bd
or
Trimethoprim 200mg bd
Three day course
Empirical treatment of uncatheterised male
Same as for female, but seven day course
Empirical management of complicated UTI/pyelonephritis/urosepsis
IV amox + gent
Step down to PO co-trimaxazole or as per sensitivities
Total 7 days
What test(s) should be considered in men with symptoms of a lower UTI?
Prostate specific antigen and DRE
Complications of UTI specific to men?
Prostatitis
Prostatic abscess
