When you know pt is in VF/pVT, what are the first few steps you take?

A
  1. shock
  2. resume CPR
  3. IV/IO access
  4. Two minutes later, check rhythm for 10 seconds max. If it is shockable, then shock the patient

When do we give Epi? Dose?

A

-Between second/third shock
-1 mg IV every 3-5 minutes (SLAM IT)

After the first dose of EPI, explain what you would do

A
  1. Give epi
  2. Two minutes of CPR, conduct another rhythm check → reads shockable → resume CPR while defibrillator charges → shock → CPR for 2 minutes

When can we administer amiodarone/lidocaine? Dose?

A

After the third shock
-Amiodarone: 300 mg IV bolus → 150 mg IV bolus
-Lidocaine: 1-1.5 mg/kg IV first then .5-.75 mg/kg IV at 5-10 minute intervals (max dose of 3)

If the rhythm is not shockable, what would you do?

A

if there are no signs of return of spontaneous circulation (ROSC) –> PEA/asystole pathway
