What must be done before starting a physical exam?
Wash/sanitize hands before and after, dress professionally, gather all equipment.
What are the steps in patient introduction and identification?
Greet, introduce yourself, confirm full name, age, gender, and gather demographic info.
How is BP measured and what does it assess?
Sphygmomanometer, upper arm, after 5 min rest. Assesses hypertension/hypotension.
How is respiratory rate measured?
Count chest rises for 30 sec x 2; assess breathing function.

Palpate radial pulse 30 sec x 2; check rhythm, strength, rate.
