Chicken pox

A

Childhood rash
Easy in child
Transmitted by raspiratory then gi to blood stream viremia then develop rash
Then cause latency in neuron
Vesicle progression(macules—>papules—>vesicles—>pustules—>crusts)
Fever and rash at same time
More on trunk than on extremities
Scars may develop
Becomes latent in dorsal root and cranial nerve gangalia

Another type of VZV is shingles

A

Shingles(herpes zoster)
Immune depression
Infect skin and causes rash along dermatome
Unilateral
Around the rib
If low immunity; shingles is very bad causing postherpatic neuralgia
Recombinant shringrix vaccine available
If you had a chicken pox vaccine you do not get shingles
Shingles is not transmitted
Only chicken pox

EBV

A

Respiratory tract and salivary glands
Sharing toothbrushes
EBV uses compliment receptor called CD21
This receptor the virus will attach to
The receptor CD21 is on epithelial cells and B cells

Causes lytic infection on epithelial and lytic and latent in B cells
Kissing disease
Can cause in rare cases cancer (lymphoma)
Shed asymptomatic for life

Disease mechanism of EBV

A

It is normally asymptomatic
But it is symptomatic when overactive immune response called mononucleosis
Symptoms of mononucleosis:
Lymphadenopathy
Fever
Pharyngitis (sore throat)
Splenimegaly
Mostly in adolescence will experience symptoms
We ask for blood work:
Lymphocytosis: increase number of lymphocytes
Atypical lymphocyted: downy cells overly activated t cells
Monospot test: transient heterophile igM antibodied

Inadequate immune response that cant fight(get b cell lymphoma)
Absence of T cell response cant fight infected B cells

Transient heterophile igM antibodies

A

We have different B cells with diff idiotyoes
When b cells are infected they start dividing and secreating IgMs
The IgM have same isotyoe Fc but differet variable regions( diff idiotypes) Fab
Then add ti a sheep RBC —> it forms a clump (positive antibody test)called monospot test

CMV

A

Viral cause of congenital defect( mind retardation, eye problems…)
Birth defects
It is only symptomatic in babies causing cytomegalic disease
It is only serious in aids
Shed by all body secretions
Give CMV negative blood for immunosupressed and pregnant women
And CMV positive for all others
Results in heterophile negative mononucleosis
Diagnosis: owls eye inclusions

Roseola(HHV-6)

A

Childhood disease
3childhood rashes: B19(rash on face)-chicken pox(VZV)-roseola(rash on trunk)
All go in respiratory then viremia then rash
High fever then disappear then rash on trunk
Persists in life
No vaccine
