Explain the CCM and the ECCM

A

CCM - focuses on the combination of the individual and the community to create productive outcomes in chronic illness; focuses on having a better prepared patient and a prepared healthcare team

ECCM - focuses more on the community and building public policy; wants to activate the community and create a supportive community (d/t the Charter)

Assist - in helping the patient cope, problem-solve, and utilize their available resources

Arrange - follow-up care, staying in contact, updating the plan as needed
