What is the main aim for the SAGO chart?
To detect deterioration and then activate healthcare interventions to prevent further deterioration.
What is the yellow and red zone in the SAGO chart?
Yellow = Early warning sign of deterioration and criteria for clinical review may be required.
Red = Late warning sign of deterioration, which a rapid response is required.
What is the response for being in the yellow zone in the SAGO chart?
- Repeat and increase frequency of obs
- Consult team leader / nurse in charge
- Complete A-H assessment
What is the response for being in the red zone in the SAGO chart?
Rapid response, a call must be made.
What are examples of early warning signs ( yellow zone ) for adult deterioration ?
- Poor peripheral circulation
- Excess/ increasing blood loss
- Decrease LOC or new onset confusion
- Low urine output <100 ml over 4 hours
- New/ increasing pain
- BGL below 4 mmol or above 20 mmol
- Concerns from the patient or family member
- Concerns from you or any staff member
What are examples of late warning signs ( red zone ) for adult deterioration ?
- All cardiac or respiratory arrests
- Airway obstruction or stridor
- Unresponsive
- Any observation in the red zone
- Deterioration not reversed within 1 hour of a clinical review
- Seizures
- Any concerns by the patient or family member
- Any concerns from any staff member
When are obs to be completed?
6 hourly intervals , 4 x per day
What are obs? (8)
- Respiratory rate
- Oxygen saturations
- Heart rate
- Blood pressure
- Temperature
- Level of consciousness
- New onset confusion or behaviour change
- Pain score
What is appropriate clinical care?
- consider A-H assessment
- call for help
- repeat vital signs
- If the patient isnt responding, initiate basic life support
What is basic life support?
DRSABCD
D = Dangers ?
R = Responsive?
S = Send for help
A = Open airway
B = Normal Breathing ?
C = Start CPR
D = Early Defibrillator
Continue CPR until responsive or normal breathing returns
What is a bag valve mask used for?
To provide positive pressure ventilation to patients who are not breathing or not breathing adequately. (in between CPR)
