what are the two types of periodontal disease
gingivitis and periodontitis
gingivitis
mildest form of periodontal disease, limited to gingiva and reversible
periodontitis
severest form, affects tooth-supporting structures, not reversible
non specific plaque hypothesis
quantity of plaque was responsible for periodontitis, thought that gingivitis always lead to periodontitis
specific plaque hypothesis
quality of bacteria is more important
what is a favorable environment for bacterial plaque
soft deposits
what is direct toxicity caused by
toxins and enzymes produced through bacterial metabolism from supra or subgingival plaque
what is indirect toxicity
when subgingival bacteria act as antigens and localized immune reaction occurs, the host response attempts to control the attack and destroys the tissue in the process
initial lesion
2-4 days, no clinical change, edema, gcf increase
early lesion
4-7 days, more wbc come, breakdown of collagen fibers, clinically there is slight enlargement
established lesion
14 or more days, more wbc (plasma cells), bop, gingival pocket, clear evidence of inflammation, still reversible
advanced lesion
transition to periodontitis, diagnosis is made by if the je is apical to cej, not reversible, bone/attachment loss
what gets destroyed in periodontitis
cementum, periodontal ligament and alveolar bone
