-care provided by medical specialists and other healthcare professionals who generally do not have first contact with patients (usually have been referred to by a GP)
-generally provided in hospitals (there are exceptions such as psychiatry)
-it may be unplanned emergency care or surgery, or planned specialist medical care or surgery
