WK8 tutorial Flashcards
(23 cards)
What is MoCA?
Cognitive assessment
* 26/30 is normal
* anything under is impairment
What are the 3 levels of impairments in clinical diagnosis of neurodegen. disease?
- SCD
- MCI
- Dementia
What can shuffling gait suggest in a physical exam?
Vascular Dementia, Lewy, PD
What can restricted upgaze suggest in a physical exam?
PSP
What can increased tone suggest in a physical exam?
Vascular dementia, Lewy, PD
What are some non-specific factors to look out for in clinical diagnosis?
- Weight loss
- Depression
- Active hallucinations/delusions
- Comorbidities - heart failure, COPD,
What is Vascular Dementia?
When conditions such as blood clots restrict blood flow to brain
What are the risk factors for vascular dementia?
- Hypertensions
- Elevated chol
- Stroke
Symptoms of vascular dementia
Clinically present same as AD, but early impairment in judgement and exec. function
What is behavioural disinhibition
Lack of control on behaviour
* Socially innapropriate
* Loss of manners/decorum
* Impulsivity
What is the neuropathology of Lewy body dementia?
alpha synuclein accumulation
Symptoms required for Lewy body
Memory impairment with +⅔ more features:
1. Visual hallucinations
2. Fluctuating cognition
3. Parkinsonism
SCD
Subjective cognitive decline
* Patient has concerns
* Everyday function is preserved
* MoCA normal
MCI
Mild cognitive impairment
* Memory tests indicate impairment
* Everyday function still preserved
Dementia (classification of impairment)
Significant cognitive decline
* No longer able to do everyday activities
What is an example of a registration and recall test?
- Tell them to remember 3 objects
- Distract them
- Come back later and ask
What is an example of a judgement test?
- Given a scenario
- Ask how they would approach the situation
- Mark against typical response
The Clock-Drawing Test (CDT)
Assess executive function and visual-spatial function
MCI management
- Treat reversible causes (e.g. meds)
- General counselling
- Offered 12-18m follow up
Drug options in AD
- Anti Amyloid
- NMDA antagonist
- Cholinesterase inhibitor
Drug options Lewy
Cholinesterase inhibit
Drug options in Parkinson
Trial of levodopa
Non drug options from neurodegen treatment
- Driving safety
- Aged care planning
- Support groups
- Lifestyle changes
