ENGINE COMPARTMENT FIRE ON DECK

A
Symptoms:
● Engine FIRE light illuminated
● Hand or voice signals from ground crew
● Red warning light illuminated on the corresponding EFSL
Corrective action (both engines):
1. CONFIRM FIRE
2. FCSs - OFF
3. EFSLs - OFF
4. BOOST PUMPS - OFF
If fire confirmed (affected engine):
5. PRI FIRE EXTINGUISHER BUTTON - CONFIRM; PUSH
6. SEC FIRE EXTINGUISHER BUTTON - CONFIRM; PUSH
7. EMERGENCY ELECTRICAL CUTOFF
8. ROTOR BRAKE
9. EVACUATE

ENGINE COMPARTMENT FIRE INFLIGHT

A

Symptoms:

● Engine FIRE light illuminated
● Red warning light illuminated on the corresponding EFSL
● Flames and/or smoke coming from engine compartment

Possible symptoms:
● ENG1 FUEL P , ENG2 FUEL P
● ENG1 OIL P , ENG2 OIL P
● ENG1 OIL TEMP, ENG2 OIL TEMP
● ENG1 CHIP, ENG2 CHIP
● Electrical system malfunctions
Corrective action (affected engine):
1. CONFIRM FIRE
                         NOTE Due to engine location, confirming a fire inflight may be difficult. It may be necessary for an aircrew member to open the cabin sliding door to observe the engine or turn the aircraft to look for a smoke trail.
  1. SINGLE-ENGINE PROFILE - ESTABLISH
    If fire confirmed (affected engine):
  2. FCS - CONFIRM; IDLE, CONFIRM; OFF
  3. EFSL - CONFIRM; OFF
  4. BOOST PUMPS - CONFIRM; OFF
  5. PRI FIRE EXTINGUISHER BUTTON - CONFIRM; PUSH
  6. SEC FIRE EXTINGUISHER BUTTON - CONFIRM; PUSH
    If fire still persists:
  7. LAND/DITCH IMMEDIATELY
    WARNING
    The potential loss of the airframe after aircrew egress
    is not sufficient cause to continue flight.
    If fire no longer evident:
  8. LAND AS SOON AS POSSIBLE
    If fire not confirmed:
  9. LAND AS SOON AS PRACTICABLE

ENGINE POST-SHUTDOWN FIRE

A

Symptoms:
● TOT rises rapidly after FCS has been placed in the OFF position
● TOT does not decrease after FCS has been placed in the OFF position
NOTE
● If the engine does not stop immediately (solenoid valve failure), the FADEC will shut down the engine 5 to 6 seconds later.
● After engine shutdown is complete and N1 rotation has ceased, TOT may increase slowly due to temperature soak-back.
Corrective action (affected engine):
1. EFSL - CONFIRM: OFF
CAUTION
Operation of the crank button with the EFSL pulled
will cause severe damage to the engine fuel pump.
If high TOT still evident after 10 seconds:
2. CRANK BUTTON - DEPRESS
If TOT continues to rise or does not decrease:
3. EMERGENCY ELECTRICAL CUTOFF
4. ROTOR BRAKE
5. EVACUATE

MGB FIRE

A

Symptom:
● MGB FIRE light illuminated
Possible symptoms:
● Illumination of associated MGB related warnings and cautions
- MGB OIL P
- MGB OIL TEMP
- MGB PMP MAIN
- MGB PMP AUX
● Loss of MGB oil pressure and/or rise in temperature
● Drop in fuel and/or hydraulic pressure
● AC electrical system malfunctions
WARNING
● Due to the potential for fire damage to the flight control servos and/or the associated hydraulic lines, a fire in this compartment may result in complete loss of aircraft control.
● Illumination of the MGB FIRE light may indicate a fire, overheat condition, or a short circuit in the detection system.
● Since no fire extinguishing system is provided for the MGB compartment, immediate action should be taken to confirm the presence of fire.
Corrective action:
1. CONFIRM FIRE
If fire confirmed:
2. LAND/DITCH IMMEDIATELY

                            WARNING The potential loss of the airframe after aircrew egress is not sufficient cause to continue flight. If fire not confirmed: 3. LAND AS SOON AS POSSIBLE

INTERNAL FIRE (CABIN, ELECTRICAL, AND/OR AVIONICS)

A

Symptoms:

● Smoke/fumes/sparks originating from a single component inside the cabin
Corrective action:

  1. CONFIRM/SUPRESS FIRE
  2. AFFECTED EQUIPMENT - OFF
  3. AFFECTED CIRCUIT BREAKERS - PULL
                         WARNING If the crew determines that the fire source is in the avionics rack, the avionics rack blower circuit breaker shall be pulled. This is located on the avionics rack panel R9 No 2.
    
                            NOTE An avionics fire will be fought by disconnecting enough camlocks on the avionics rack panel to allow access for the fire extinguisher nozzle. Short blasts are advised to preserve extinguishing agent in case of a reflash. Close rack panel and monitor for reflash
  4. HEAT/COOL SWITCHES - OFF
  5. RAM AIR, DOOR, WINDOW - CLOSED
  6. WHEELS/FLOATS - AS REQUIRED
                        WARNING ● The severity of the fire and actual flight conditions (night/instrument) will dictate the immediate procedures to be followed. Although the SFI will be powered, it may not be advisable to secure all electrical power, thus losing AFCS and MFDs, prior to achieving VMC.

● With the EMERGENCY ELECTRICAL CUTOFF switch in the OFF position, the flotation system will not be available. Consideration should be given to activating emergency floats prior to securing if ditching is anticipated.

● All communication, both internal/external, and all aircraft system lighting will be lost after activating EMERGENCY ELECTRICAL CUTOFF switch.

● Placing the EMERGENCY ELECTRICAL CUTOFF switch to OFF removes power to the tail rotor hydraulic isolation valve, closing the valve. This action may result in considerable feedback in the pedals.
If electrical or avionics fire persists:

  1. EMERGENCY ELECTRICAL CUTOFF

If electrical or avionics fire persists:

                   WARNING The potential loss of the airframe after aircrew egress is not sufficient cause to continue flight. If fire goes out: 9. LAND AS SOON AS POSSIBLE

SMOKE AND FUME ELIMINATION

A

Corrective action:

                     WARNING If fuel fumes are present, limit radio transmissions to a minimum. Due to antenna location, COMM1 is the best choice.
  1. HEAT/COOL SWITCHES - OFF
  2. RAM AIR, DOOR, WINDOW - OPEN
  3. LAND AS SOON AS PRACTICABLE
                         NOTE ● The SEAS bottle, located in crew survival vests, may be a good source of clean air in the event of an unbreathable environment. ● Normally, no toxic quantities of carbon monoxide gas or other gases are present from the engine exhaust. Objectionable odors from the ECS are sometimes experienced due to internal oil leaks in the engines and should be entered in ALMIS.

BATTERY OVERTEMPERATURE (AIRCRAFT OR SFI)

A
Possible symptoms:
● BATT TEMP
● SFI Battery FAULT light
● Smoke and/or fumes from radome or vent
● Noises from radome
If the BATT TEMP is present

Corrective action:

                   WARNING Securing the BATT RELAY switches will not stop or reverse a battery over temperature/thermal runaway.

                   CAUTION If BATT TEMP warning is no longer displayed, BATT RELAY switches should remain off unless battery power is absolutely needed.
  1. BATT RELAY SWITCHES - BOTH OFF
                         NOTE ● In the event of dual DC generator failure, the T/R cannot power either main DC bus with the battery switches secured. ● A No. 2 DC generator failure with the No. 2 BATT RELAY switch in the OFF position will create a SEC HYD ISOLATE condition and result in considerable feedback in the pedals.
  2. SFI ACFT - OFF
                            NOTE The backup battery will power the SFI for a minimum of 30 minutes.

If BATT TEMP is not present and the SFI FAULT light is illuminated with smoke
from radome.

  1. SFI BATT and ACFT - OFF

If symptoms persist (smoke/fumes from radome or electrical malfunctions):

  1. LAND AS SOON AS POSSIBLE
After landing (If BATT TEMP light remains illuminated, thermal runaway
suspected):
  1. Allow battery to cool prior to removal
  2. Secure and exit aircraft
  3. Stand by with fire fighting equipment and personnel capable of fighting the fire
                      WARNING CO2 should never be directed into the battery compartment to effect cooling or displace the explosive gases. The static electricity generated by CO2 could cause the hydrogen/oxygen gases in the compartment to explode.

If symptoms extinguish inflight:
