JVD 2020 #1 Transcutaneous Lateral Alveolar Ostectomy for Standing Surgical Extraction of Mandibular First Molar in an 8-Year-Old Miniature Horse; Limone Flashcards

1
Q

What is partial coronectomy?

A

Intraoral extraction technique that involves removal of clinical crown of the diseased tooth in the interproximal space. The increased interproximal space allows access for luxation and axial rotation of the diseased tooth.

Q

How is crown sectioning used for equine extraction?

A

Useful to turn a multi-rooted extraction into extraction of single-rooted crown-reserve crown fragments.
Reserve crown or root fracture is likely to occur with oral extraction of a tooth with dilacerated roots. Sectioning of the clinical and reserve crown to the level of the furcation in a buccal to lingual direction allows extraction of mesial and distal crown root segments separately.

Q

What are the disadvantages to partial coronectomy and sectioning techniques?

A

Potential to damage adjacent healthy teeth and loss of crown integrity leading to retained roots which may be difficult to extract.

(can cause damage to MN and adjacent structures)

Q

Minimally invasive transbuccal screw extraction (MITSE)

(not used in this report)

A

Provides direct access to the target tooth using straight instruments for elevation and luxation. Screw placement in the tooth allows for extraction of teeth that would not be extractable with forceps. Indicated for fractured or absent clinical crowns, malformed or dislocated teeth.
Poss drawback- inability to extract due to screw thread stripping or continued tooth fracture.

Q

What abnormalities are seen in this open-mouth right ventral to left dorsal lateral oblique view of the L MN cheek teeth?

A

Radiopaque probe inserted into the draining mandibular fistula to confirm communication with 309

309 periodontal ligament widening and osteosclerosis along the mesial reserve crown root and mesial aspect of the distal dilacerated root

Radiolucent area present within the pulp horns and pulp chamber of the reserve crown just coronal to the furcation was associated with a mesial bulge of the reserve crown

Periapical alveolar bone lysis along the mesial root tip
