over by CHP driver.
(d) Confused driver collides with third vehicle while being signaled by
CHP driver.
(e) Collision is intentional – to stop violator (legal intervention) unless the
commander determines the action was ill-advised.
(f) CHP vehicle flips rock, causing damage to other
vehicle. NOTE: Completion of the CHP 555/556, Traffic Collision Report, and
entry on the CHP 442, Individual Accident, Injury, and Safety Recognition
Record, are not required when the accident is non-recordable.

Category A:
1) Any incident in which a member of the Department is seriously injured or killed in the performance of his duties if his/her duties.
2) Any incident involving the use of force by a member of the Department,

or bystander.
3) Any in-custody death incident.
4) Any incident involving a patrol vehicle collision which results in death or
serious injury to a party or parties, other than a member of the department
when the liability appears to be that of the Department.
5) Special relationship incidents resulting in serious injury and/or death.
The policies and procedures contained in GO 100.80, Report In conjunction with of
Unusual Occurrences, shall be followed. the notification of the appropriate Assistant Commissioner, ORM ______ be notified as follows:
