Symptoms of Opioid Intoxication
-Euphoria and Sedation
-Slow, slurred speech
-Pupillary constriction (narcotics are miotics)
-AMS
-Respiratory depression
-Bradycardia
-Hypotension
Opioid Withdrawal
-Lacrimation
-Hypertension
-Pruritis
-Pupil dilation (mydriasis)
-Flu-like symptoms: rhinorrhea, joint paints, myalgias
-Piloerections (goosebumps)
-Tearing
Management for Opioids
-Intoxication
-Withdrawal
-Dependence
-Intoxication: Naloxone
-Withdrawal: Clonidine
-Dependence: Methadone, Naltrexone, Buprenorphine
Alcohol Withdrawal
-Uncomplicated
-Withdrawal Seizures
-Alcoholic hallucinosis
-Delirum Tremens
Uncomplicated: 6-36 hours after last drink. Increased CNS activity (tremors, anxiety, irritability, GI, insomnia. NO SEIZURES OR HALLUCINATIONS.
-Seizures: 6-48 hours after last drink. Tonic-clonic type.
-Hallucinosis: 12-48 hours after last drink. Clear sensorium and normal vital signs.
-Delirium Tremens: 2-5 days after last drink. Delirium, abnormal vital signs, diaphoretic.
Treatment for alcohol withdrawal
-Can be fatal
-IV Benzodiazepines
-IVF, IV Thiamine (B1), magnesium, multivitamins, electrolyte repletion
Cocaine Intoxication Symptoms
-Elevated or euphoric mood
-Pressured speech
-Nausea, vomiting, seizures
-Pupillary dilation (mydriasis)
-Respiratory depression, seizures, repetitive behaviors
-Aggression, Agitation
Treatment for cocaine intoxication
-Benzos for mild
-Severe or psychosis: Haloperidol, DO NOT PLACE IN RESTRAINTS –> rhabdomyolysis
Symptoms of PCP Intoxication
-Rage, fear, impulsivity
-Hallucinations
-Multidirectional nystagmus
-Ataxia, tachycardia
Treatment for PCP Intoxication
-Supportive: airway, breathing, circulation
-Benzodiazepines if agitated, hypertensive, seizures
Symptoms of cannabis intoxication
-Euphoria
-Anxiety, dry cotton mouth
-Increased appetite
-motor impairment
-Fear/depression
-Psychosis
-Conjunctivitis
-Tachycardia, hypotension
Wernicke’s Encephalopathy
-Triad of ataxia, confusion, and oculomotor palsy (due to thiamine/B1 deficiency)
Korsakoff Syndrome
Amnesia (both retrograde and antegrade)
–past memory and can’t form new memory
What is used to treat benzodiazepine intoxication?
