Primary Health Care

A

A basic level of health care that includes programs directed at the promotion of health, early diagnosis of disease or disability, and the prevention of disease. Primary health care is provided at an ambulatory facility to limited numbers of people, often those living in a particular geographic area. It includes continuing health care, and provided by a family nurse practitioner.

A

An intermediate level of health care that includes diagnosis and treatment. It is usually performed in a hospital having specialized Equipment and labratory facilities.

Tertiary Health Care

A

A specialized, highly technical level of health care that includes diagnosis and treatment of disease and disability. Specialized icus, advanced diagnostic support services, and highly specialized personnel are usually a characteristic of tertiary health care.

Acute Care

A

A pattern of health care in which a patient is treated for a brief but severe episode of illness, for the sequelae of an accident or other trauma, or during recovery from surgery. Acute care is usually given in a hospital by specialized personnel using complex and sophisticated technical equipment and materials, ad it may involve intensive care, critical care, or emergancy care.

A

The activities that facilitate a patient’s movement from one health care setting to another, or to home. It is the multidisiplinary process involving physicians, nurses, social workers, and possibly other health professionals; it’s goal is to enhance continuity of care.

Medicare

A

A federally funded national health insurance program in the United States for people over 65 years of age or who meet other criteria. Part A provides basic protection against costs of medical, surgical, and psychiatric hospital care. Part B is a voluntary medical insurance program financed in part from federal funds and in part from premiums contributed by enrollees. Medicare was authorized by Title XVIII of the Social Security Act of 1965.

A

A U.S. federally funded state-operated program of medical assistance to people with low incomes, authorized by Title XIX of the Social Security Act. Under broad federal Guidelines, the individual states determine benefits, eligibility, rates of payment, and methods of administration.

A

An institution or part of an institution that meets criteria for accreditation established by the sections of the Social Security Act that determine the basis for Medicaid and Medicare reimbursement for skilled nursing care. Skilled nursing care includes rehabilitation and various medical and nursing procedures. Written policies and protocols are formulated with appropriate professional consultation.

Minimum Data Set (MDS)

A

Required by the Omnibus Budget Reconciliation Act of 1987, the MDS is a uniform data set established by the Department of Health and Human Services. The MDS serves as the framework for any state-specified assessment instruments used to develop a written and comprehensive plan of care for newly admitted residents of nursing facilities.

A

A payment mechanism for reimbursing the hospital for inpatient health care services in which a predetermined rate is set for the treatment of specific illnesses. The system was originally developed by the U.S. federal government for use in the treatment if Medicare recipiants.

A

A group of patients classified for measuring a medical facility’s delivery of care. The classifications, used to determine Medicare payments for inpatient care, are based on primary and secondary diagnosis, primary and secondary procedures, age, and length of hospitalization.
