Who completes the UOF reports on OIS’s, use of deadly force incidents, and some pursuits?

Can a supervisor involved in a UOF incident investigate other involved officers?
No, investigative duties will be relinquished to the next level of supervision.
If a supervisor determines substantial injury has occurred as a result of a UOF, or if deadly force was used, they will:
1) Ensure medical attention is provided for those in need.
2) Ensure the crime scene is safe and secure, preventing the possibility of contamination or alteration.
3) Update Dispatch and request that notifications be made.
4) Coordinate the search for outstanding suspects.
5) Oversee the identification and isolation of witnesses.
6) Ensure witness officers and subject officers are separated and placed in a secure environment with a monitor officer.
7) Provide FIT detectives on the names and locations of the subject officers and potential witness officers.
8) Remain on scene, and do not leave until released by the FIT supervisor.
Who reads the PSS?
A supervisor. If none available, the senior officer on scene.
Should PSS’s be recorded on BWC?
No
How should PSS responses be recorded?
Handwritten by supervisor/senior officer asking questions.
Where do photographs of UOF scenes get attached?
In the Blue Team report.
What happens to taser probes after discharge?
Handled as contaminated needles. All probes, wires, and cartridges are impounded as evidence.
Non-deadly UOF with vehicle: Who assists with investigation?
Traffic officers
Who investigates the use of non-deadly force?
The officer’s chain of command.
What is the lieutenant’s responsibility when reviewing Blue Team reports?
They will: “Review the supervisor’s investigation and all attached documentation, to include video footage, and evidence in Blue Team to ensure accuracy and completeness.”
Public Safety Statement (PSS):
A series of questions to obtain information to determine if there is an immediate threat to public safety.
Reportable Force:
Any use of force which results in injury or complaint of injury, complaint of continuing pain, or any use of force greater than low-level force.
Levels of control which require completion of a UOF report:
- Empty hand tactics with injury or complaint of injury
Baton/Impact tool
OC Spray
ECD, including spark display
Canine - with bites
Use of force with vehicle
Use of force with projectile weapon - LL/40mm
Use of force with firearm
Force that results in injury, complaint of injury, or complaint of continuing pain
Post UOF supervisor responsibilities(on scene):
Respond to the incident without delay.
Determine level of force. If deadly/substantial, make notifications.
Conduct an investigation by interviewing subjects, officers, medical personnel, and witnesses.
Ensure photographs are taken and placed into Blue Team.
Review BWC (when applicable).
Check surrounding areas for video, obtain copy, impound and notate in BT.
Review all applicable reports to ensure accuracy and completeness.
- Document if the force is appropriate
- If not appropriate, forward for determination if SOC should be opened
- Document training deficiencies and how they were addressed
ECD - upload data record into BT, verify probes/wires/cartridges are impounded.
