What is dermatitis herpetiformis?

A

A chronic, autoimmune, blistering skin disease with very itchy papules and vesicles

What is the pathophysiology of dermatitis herpetiformis?

A
  • Strong genetic predisposition
  • HLA-DQ2, HLA-DQ8
  • Main environmental trigger is ingestion of gluten (anti-tTG antibodies form)

How does DH present?

A
  • Symmetrical, erythematous, itchy papules usually on the elbow, knees and buttocks
  • Can present on scalp, face and neck
  • Episodes of exacerbation and remission

How is DH investigated?

A
  • Skin biopsy which shows IgA deposits
  • Serology can detect the anti-tTG antibodies
  • Screening for coeliac disease is recommended

How is DH managed?

A

Lifelong gluten free diet
Dapsone can be given for symptomatic relief
