Blood Alcohol violation levels:
0.04% Non-DOT

List 3 situations that may require Drug/Alcohol testing:
- Reasonable suspicion, or post-accident or injury
- Layoff/absence of 30 days or more
- Safety sensitive positions subject to random testing
Employee may request retest of original specimen within:
72 hours of notification
At employee expense
If alcohol test isn’t administered within 2 hours following incident:
Immediate supervisor and department head must record reason and forward it to HR
Alcohol test not administered within 8 hours:
No attempt will be made. Immediate supervisor and department head will record reasons and forward to HR
Controlled substance test not administered within 32 hours:
No attempt will be made. Immediate supervisor and department head will record reasons and forward to HR
