What is the definition of a ‘pyrexia of unknown cause’?
A temperature measured >38c for 3 weeks with 3 unsuccessful attempts of diagnosis
What is defined as a high grade fever?
> 40c
Which infections present with an intermittent fever?
TB
Malaria
What is a remittent fever?
A fluctuating fever that remains high
Which infections present with a remittent fever?
Infective endocarditis
Rickettsia
What are the associated symptoms with fever to ask for?
Fatigue
Rash
Weight loss
Night sweats
Rigors
Lymphadenopathy
What are the most common causes of PUO?
Infective:
- TB (extra-pulmonary)
- Intraabdominal abscess/ pelvic abscess
Inflammatory:
- Vasculitis
- SLE
Malignant:
- Leukaemia
- Lymphoma
What disease is sterile pyuria associated with?
Genitourinary TB
- Send urine sample to micro for mycobacteria testing
Also STI’s, cystitis, and bladder cancer
What is sterile pyuria?
WBC’s in the urine without any bacteria
How does slap-cheek syndrome present?
Caused by parvovirus B19
- A non-tender, warm rash
- Bright red cheeks
- Anaemia
- Severe in adults
Which virus causes and, foot, and mouth disease?
Cocksackie virus
What is erythema nodosum?
A manifestation of infection/ inflammatory disease
Causes painful patches usually bilaterally
What is erythema multiform?
An allergic reaction to meds
Often seen following vaccines in infants
What is panniculitis?
Inflammation of the subcutaneous fat
Can look like cellulitis, but is bilateral and not oedematous
What is Ramsey-Hunt syndrome?
A facial nerve palsy (and hearing loss) resulting from shingles in that area
