Manner not authorized by FDA. Indications and dosages not on labels. No illegal- can Rx if appropriate standard of care. Can be sued if errors occur with off label use

Common causes of med errors
Sound alike drug names, lack of knowledge, dose calculations, decimal placement, wrong dosage, frequency, overlooked interactions, failure to adjust in renal disfunction, incomplete pt hx.
