Dabigatran

A

Name:Dabigatran (Pradaxa)

Rivaroxaban (xaralto)

Apixaban (eliquis)

Class: oral direct thrombin inhibitor

MA: The monovalent direct thrombin inhibitors (argatroban, dabigatran, ximelagatran) all bind the active site on thrombin, preventing it from activating fibrin and completing the cascade

CI: 1)high risk of bleeding- uncontrolled active bleeding, major bleeding, acute gastric or duodenal ulcer, recent stroke (<6 months)

SE: bleeding

Indications:stroke prevention in AF, VTE tx, VTE prophylaxis, post TKA/THA surgery

THERE IS NO ANTIDOTE FOR THE DIRECT THROMBIN INHIBITORS

Dosing of dabigatran

AF: 110-150mg po BID

VTE prophylaxis: 220mg po OD
