What is croup?
Most common infection of the middle respiratory tract in children
When does croup generally present and in what age group
Fall- early winter. 6 months-3 years
What are common causes of croup?
Nearly all viral. Parainfluenza virus Types 1,2,3, influenza, RSV
What is a classic sign of croup on xr?
Steeple sign
What are clinical manifestations of croup?
Onset typical URI symptoms (cough, low grade fever, rhinorrhea) hoarse voice, stridor with coughing or crying, classic barky cough, retractions
What is indicates that the child is getting worse?
Stridor at rest
What is laryngomalacia/Tracheomalacia?
Developmental disorder of airway. Cartilaginous structures of larynx and or trachea too soft and collapse with inspiration
What is characteristic of airway obstruction from mass?
Insidious onset (weeks to months)
What are types of airway obstruction from mass?
Hemangioma, tumor (neuroblastoma), retropharyngeal abscess
Bacterial tracheitis?
Infection of trachae most commonly by staph aureus
What is the treatment for croup?
Observation with mild illness/older child/ Oral dexamethasone single dose or prednisolone several doses. Racemic epinephrine for respiratory distress
Why should epinephrine and steroid be given together in ER?
Possible rebound since epinephrine wears off in 60-90 mins. May need to repeat every 30-60 mins until steroid kicks in.
