What is the Ortolani test?
Abduction and relocation of the dislocated hip
What is the Barlow test?
Adduction and dislocation of the hip
When would ortolani and Barlow tests be positive?
Only in the neonate
How should a diagnosis of DDH be confirmed in a child aged up to 3-6 months?
Ultrasound
How should a diagnosis of DDH be confirmed In a child >6months?
X-ray - due to calcified epiphysis
How should DDH be treated In children up to 6 months?
Splint - pavlik harness - abduction and flexion
What treatment of DDH may be necessary in children over 2 years?
Surgery - osteotomy
How might an older child present with DDH if not diagnosed as a neonate?
A limp ever since they started walking
What are the 4 main signs of septic arthritis in a child?
Inability to weight bear
Fever >38
ESR >40
WCC >12
What may precede transient synovitis?
URTI
What is Perthes?
Idiopathic avascular necrosis of the hip
What ages does Perthes typically present in?
3-12
What is the typical child who gets Perthes?
Small active boys
How is Perthes managed?
Younger children, milder cases - Non operatively, limit activity
Older children, severe cases -femoral or acetabular osteotomy
What is osteochondritis dissecans?
Avascularity of the lateral side of the medial femoral condyle
What symptoms are present in osteochondritis dissecans?
Intermittent locking and pain
What is osgood-schlatters disease?
Idiopathic aseptic necrosis of the tibial tuberosity - knee pain in adolescence
What age does SUFE typically present?
10-16 years
What groups are more likely to get SUFE?
Overweight boys
Where might the pain of SUFE be felt?
In the knee
What investigations should be performed in suspected SUFE?
LATERAL x-ray
What is the main complication of SUFE?
AVN - 47%
What should be ruled out in any adolescent who is unable to weight bear?
SUFE - do not weight bear until x-ray is performed
