Outline the SAP and the 3 decision tools

A

The single Assessment Process also referred to as multi disciplinary assessment.
Provides a consistent approach in relation to assessing both Health Needs and Social Care needs
The decision tools
1 Checklist - screening tool
2 A decision Support tool - full assessment and DST will be completed
3 a Fast Track Tool - enable care package within 48 hours for someone extremely ill/terminally ill

Once assessed will be documented in a Care Plan as result of Care Act 2014

A

Known as fully funded care where individual has a ‘primary health need’

1 Respite Care
2 Palliative Care
3 rehabilitation care

Outline the National Framework for NHS Continuing Care

A

Decision should be transparent, fair and consistent
Fully funded by NHS
Decision Support Tool in relation to 12 care domains include: mobility, nutrition, breathing,
NHS con Care each domain has 6 levels rating from no needs, low, moderate, high, severe.
Only 4 domains can be a * above
