Usually it starts with a introduction or background of the project, in scope, out of scope items, different types of testing involved (smoke, functional, regression, performance), entrance and exit criteria, high level approach to testing, test schedule, testers roles & responsibilities, testing tools and techniques, test deliverables, Issues/Risks and Contingency plans, test metrics/KPI to be produced
