angina
Disorder where there is plaque formation in the coronary arteries
–Decreased blood flow and thus decreased oxygen to heart muscle = chest pain
anti anginal drugs act by
dilating coronary arteries, thus increasing blood flow and oxygen to heart muscle = pain subsides
nitrates
Relaxing effect on smooth muscle of vessel = dilation = more blood flow
-Ex= Nitroglycerin (NTG), Isordil
calcium blockers
Relaxing effect on smooth muscle of arteries and arterioles = reduces workload of heart
To treat and prevent angina pectoris
- Nitroglycerine sublingual = to relieve pain of acute anginal attacks when they occur
- Prophylaxis: isosorbide (Isordil)
nitrates
-Prevent anginal pain in certain types
amlodipine (Norvasc), verapamil (Calan)
Calcium Channel Blockers
nitrate adverse reactions
-Common reaction is Headache
-Hypotension, dizziness, weakness, flushing
RX = may decrease dose
-Increased hypotensive effect when administered with antihypertensives, phenothiazines, alcohol
nursing process
Thorough description of anginal pain
-Type of pain
-Radiation
-Cause
-Relief
Vitals, lung sounds
-notify MD for pulse <60bpm
Nitrates = sublingual, oral, IV or
transdermal routetopical application
- Usually measured in inches of an ointment
- Applicator paper
- Remove previous application and clean area
- Place new dose on new site
- WEAR GLOVES = medication will be absorbed by the nurse when touched
- Time, date and initial new patch
IV NTG
infused by IV pump
-Bottle should be protected from light
PT TEACHING
- Take meds as prescribed
- Be aware HA is common Side effect
- Keep adequate supply of meds
- Lie down when taking meds
- NTG kept in dark container
- Keep record of pain and medication taken
- Be aware of expiration dates
