Q

Describe the pharmacological management of COPD

A

Inhaled therapy;

  • Short-acting bronchodilators (SABA, SAMA)
  • Long-acting bronchodilators (LABA, LAMA)
  • High dose inhaled corticosteroids (ICS)

Q

Define pink puffers

A
  • Increased alveolar ventilation
  • Near normal PaO2
  • Normal/low PaCO2
  • SOB but no cyanosis
  • May progress into type I respiratory failure

Q

Define blue bloaters

A
  • Decreased alveolar ventilation
  • Low PaO2
  • High PaCO2
  • Cyanosis but no SOB
  • Insensitive to CO2, thus rely on hypoxic drive to breath

Q

Under what terms would long term oxygen be given to a COPD patient?

A
  • PaO2 <7.3kPa (normal = 10.3-13.3)
    OR
  • PaO2 7.3-8kPa if: polychythaemia, nocturnal hypoxia, peripheral oedema, pulmonary hypertension
