Q

CA inhibitors MOA

A

Proximal convoluted tubule
Reduces HCO3 absorption
Can cause metabolic acidosis- used as a counter for respiratory/metabolic alkalosis

Q

Loop diuretics MOA

A

Thick ascending limb
Stop Na+/K+/2Cl- co-transporter
K+ normally secreted to allow electrochemical gradient for Mg and Ca to be absorbed- doesn’t happen anymore so decreased Mg2+ and Ca2+ absorption

Triamterene

18
Q

Aldosterone antagonists

A

K+ sparing diuretics
Spironolactone
Eplerenone

19
Q

Amiloride MOA

A

Collecting tubule
Blocks Na+ channel
Doesn’t affect K+

20
Q

Amiloride Clinical uses

A
Heart failure
Hypokalaemia (from other diuretics)
--> co-amilofruse
--> co-amilozide
Cirrhosis
21
Q

Amiloride Side effects

A

Hyperkalaemia

Hyponatraemia

22
Q

Spironolactone MOA

A

Collecting tubule

Aldosterone antagonist- normally aldosterone upregulates ENaCs

23
Q

Aldosterone inhibitors clinical uses

A

Hyperaldosteronism (Conns)
Heart failure
Hypokalaemia (from other diuretics)
Cirrhosis

24
Q

Aldosterone inhibitors side effects

A

Hyperkalaemia
Gynecomastia
Hyponatraemia

25
Q

Osmotic diuretics example

A

Mannitol

26
Q

Osmotic diuretic MOA

A

Osmotically active molecule in tubular lumen- stops water reabsorption
Reduces reabsorption of all solutes- Na, K, urea
Molecule drags Na from interstitial space

27
Q

Osmotic diuretics clinical indications

A

Cerebral oedema

Oliguric acute renal failure

28
Q

Osmotic diuretic side effects

A

Transient fluid overload (pulmonary oedema)

29
Q

Other non pharmacological osmotic diuretics

A

Glucose- gives DKA

Urea- fives disequilibrium syndrome

30
Q

When not to give diuretics

A
Hypotensive
Dehydrated
Hypokalaemic (loop, thiazide)
Hyperkalaemic (amiloride, aldosterone antagonists)
Post surgery with poor urine output
