describe the management of angina.
A
all patients given aspirin + statin + GTN spray
1st ine: beta blocker or CCB
2nd line: beta blocker + CCB (not verapamil as its a rate limiting)
3rd line: add 3rd drug awaiting PCI or CABG

with regards to tolerance to GTN what does the BNF advice patients?
A
take the second dose 8 hours after the first dose instead of 12 hours of isosorbide mononitrate
allows blood nitrate levels to fall for 4 hours and maintains effectiveness

if a patient with asthma has angina but symptoms aren’t controlled after the use of CCB, what would you do next to control their angina?
A
add either nicorandil, ivabradine, ranolazine, long acting nitrate
