What are the red flags in an abdo assessment
- confusion
- work of breathing
- pale
- AVPU
- Jaundice
- Obvious pulsating masses
What is included in introduction
- Introduce yourself
- Get introduction from patient
- Consent
- Ask about any pain
- Explain what the exam will entail
What is the typical presenting complaint for abdo?
- Left or right sided flank pain that can radiate inferiorly or anteriorly towards the groin area
Hx of presenting complaint
S - Left or right sided flank pain
O - How long has it been ongoing
C - severe and sudden
R - Is it radiating inferiorly etc
A - Any other symptoms (i.e. decreased urine frequency, nausea, vomiting etc).
T - how has the symptom changed over time
E - does anything make it better or worse
what is the signficance of PMH?
have they had anything like this before, is there any significant family history, any other chronic conditions or risk factors, high cholesterol etc.
What is important about medications?
are they taken as prescribed, are they over the counter, have they taken any pain relief today
do they have any surgical history?
recent surgery, scars, what for, when, outcome
social history
- any smoking
- lifestyle changes
- ## alcohol / illicit drug use
systems review?
- fits, faints, chest pain (silent MI), other signficant risk factors
what are the cardinal questions for abdo
- Urine - frequency, urgency, colour, smell, oral intake
- Stool - last motion, colour, texture, different to normal?
- Possibility of pregnancy
- Weight gain / loss
- Appetite changes?
- D and V
what should you do at the start of physical inspection
stand at the foot of the bed, quick visual inspection of any changes, get the patient into appropriate position, i.e. lying on their back
Consider:
- age
- gender
- BMI
- Ethnicity
- Morphologies
- lifestyle clues (social history)
