An infection of the BILIARY TREE
What is the main causative organism of acute cholangitis?
E-coli
What are the symptoms of acute cholangitis?
Charcot’s triad: Fever, RUQ, Jaundice
What are the causes of acute cholangitis?
Gallstones in common bile duct
Infection introduced during ERCP procedure
What is Reynold’s pentad?
Clinical signs distinguishing SEPTIC acute cholangitis
Fever, RUQ, Jaundice + Hypotension/Bradycardia, Confusion
What is the management of acute cholangitis?
Sepsis 6
Broad spectrum Abx
Anti emetics if nausea
1st line: ERCP within 24-48 hours to relieve obstruction
2nd line: PTC
What are the investigations for acute cholangitis?
Raised CRP and WCC
All raised LFTs
Gold standard: MRCP
