What is biliary colic?
Temporary obstruction of gallbladder neck by gallstone
What are the features of Biliary Colic?
Colicky RUQ pain triggered by high fat meals
Murphy’s -ve
Systemically well
What are the risk factors of Biliary Colic?
Fair, female, fat, forty, DM, Crohn’s Disease, COCP, rapid weight loss
If a patient develops jaundice following a cholecystectomy for biliary colic, what has happened?
May have had a gallstone in CBD, has developed obstructive jaundice
What is investigations of biliary colic?
CRP + WCC normal
LFTs normal
Abdo US
What is the management for biliary colic?
Conservative - analgesia eg diclofenac, buscopan (antispasmodic)
Weight loss, avoid fatty meals
Elective lap chole for recurrent attacks
Which enzyme is responsible for stimulating the gallbladder to secrete bile?
Cholecystokinin
