Unstable angina

How do you differentiate between 3 types of ACS?
A
Unstable angina - ECG changes + normal trop
NSTEMI - ST depression, t wave inversion, raised trop
STEMI - raised ST and raised trop

How is an NSTEMI diagnosed on ECG?
A
> 2mm in 2 adjacent chest leads
2mm in 2 adjacent limb leads
New LBBB

How is STEMI treated within 2 hours of onset?
A
Primary PCI
Thrombolysis if not PCI not available

Management of STEMI
A
IV morphine (plus anti-emetic) Oxygen (only if sats <95%) GTN spray Aspirin 300mg Ticagrelor 180mg or clopidogrel 75mg

A
IV morphine (plus anti-emetic) GTN spray Aspirin 300mg Clopidogrel 300mg Foondaparinux Angiography +/- PCI
