Republicans are in control of all the branches of the government; their leaders are extremely conservative and from small, regressive states that otherwise would not have much federal power

Describe the historical context of hospitals and physician training.
Hospitals were initially where people went to die. Only the rich received care in their homes. The AMA was founded and medical doctors were seen as a professional occupancy in the early 1900’s. Training was initially based on apprenticeship but the Flexner 1910 Report made medical schools more standardized.
Describe the early history of American health insurance.
Teddy Roosevelt proposed national health insurance in the early 1900’s, or what was otherwise known as the Progressive era. The AMA opposed health insurance because they perceived it as a threat to M.D. authority and income. They claimed that health insurance was a socialist ideal to convince the public that it was bad. FDR later promoted a Social Insurance model.

It is conferred by employment rather than citizenship
