Definition
Presence of endometrial glands and stroma in the myometrium making the uterus diffusely enlarged, boggy and globular
risk factors
hysterectomy, parity, age, no of abortions, surgery of uterine cavity, uterine malformation, menorrhagia
Clinical manifestations
- asymptomatic
- Dysmenorrhea
- Non cyclic pelvic pain
- Abnormal uterine bleeding
- Infertility
- Enlarged uterus
What are the 2 types of adenomyosis
- focal adenomyosis
- diffuse adenomyosis
Pathogenesis
Presence of endometrial tissue in the myometrium atleast 1 low magnification field of view from the endometrium - muscular junction zone
Histopathology
- Myometrial hypertrophy n proliferation
- High level of estrogen - stimulated hyperplasia of the basalis layer of endometrium
- barrier bw myometrium and endometrium is broken and endometrial cells invade the myometrium
Most common in which part?
Fundus and posterior uterine wall
Diagnosis
- Typical symptoms such as dysmenorrhea and heavy bleeding
- uterine enlargement in gyn examination
- ultrasound more than 80% accurate
- MRI - most accurate tool
What is the best tool to distinguish between adenomyosis and leiomyoma?
MRI
What is the definitive method for the diagnosis and treatment of adenomyosis?
Hysterectomy
Medical treatment options
- GnRH agonists
- Progestins - dienogest
- Oral contraceptives
- Danazol
Non surgical techniques
- Uterine artery embolization (UAE)
2.High intensity focused ultrasound (HIFU)
Surgical techniques
- Adenomyomectomy - double flap method
- Hysterectomy - total hysterectomy is the only definitive treatment of adenomyosis
