TCA
First generation antidepressants. End in Pramine and Tyline.
Significant first pass effect, lipid soluble and binds to proteins with slow elimination
Imipramine
TCA for enuresis (bed wetting)
Metabolism–>desipramine which makes it have less antimuscarinic activity
Amitriptyline
TCA also for chronic pain
Metabolism–>nortriptyline which makes it have less antimuscarinic activity
Clomipramine
Also for OCD
TCA MOA
Inhibit reuptake of biological amines (NE 5HT)
Block muscarinic cholinergic receptors
Block alpha1 adrenergic receptors- ORTHOSTATIC HYPOTENSION #1 limiting factor
Antidepressants BBW
Increased risk of suicide
Second Generation Antidepressants
Mirtazapine, Trazodone, buproprion
Mirtazapine
Blocks 5HT2 and Alpha2 adrenergic receptors
Very sedating and associated with weight gain
Trazodone
Inhibit serotonin reuptate, block 5HT2 receptors. Priapism side effect.
Bupropion
Weak NE, 5HT, Dopa reuptake inhibitor. Can also be used for smoking cessation. Do not give to those with seizures or eating disorders.
SSRI
Fluoxetine, paroxetine, fluvoxamine, sertaline, citalopram, escitalopram, vilazodone, and vortioexetine
SSRI side effects
Sexual and GI dysfunction. Caution with MAO drugs due to Serotonin syndrome
SNRI
Duloxetine, Ventaxine (NE at higher dose)
Non selective MAO
isocarboxazid, phenelzine, tranylcypromine
Mood stabilizers
Carbamazepine, valproate, lamotrigine, lithium
Can produce leucocytosis
Excreted through renal
Lithium and its side effects
