(23 cards)
Amphoteracin B MOA
Binds ergosterol
Tears holes in membrane allowing electrolytes to leak out
Amphoteracin B use
Serious systemic mycoses
DOA for diaper rash and vaginal candidiasis
Nystatin
Nystatin MOA
Binds ergosterol
Topical Amphoteracin B
Tx for oral thrush
Nystatin swish and swallow
–Azole MOA`
Inhibits ergosterol synthesis
inh. P-450 ensyme that converts lanosterol to ergosterol
–Azole use
local less severe mycoses
Topical –azoles
clotrimazole and miconazole
Itraconazole use
Tx blasto, cocci, histo
dimorphic
Flucytosine MOA
Inhibits DNA and RNA synthesis
conversion to 5FU by cytosine deaminase
Major use for flucytosine
cocci meningitis with amphoteracin B
–Azole toxicity
Testosterone inh.
Liver dys.
Flucytosine toxicity
bone marrow suppression
Caspo/micafungin MOA
Inh. cell wall synthesis –> inh. ß-glucan synth
Caspo/micafungin use
invasive Aspergillus and Candida
Caspo/micafungin toxicity
GI upset
Flushing (histamine release)
Terbinafine MOA
inh. fungal enzyme squalene epoxidase
Terbinafine use
dermatophytes
especially onychomycosis–nails
terbinafine tox
abn. LFTs
visual disturbance
Griseofulvin MOA
Interferes with microtubule fxn –> disrupts mitosis
where does griseofulvin deposit
keratin containing tissue (nails)
Griseofulvin use
oral tx of superficial infxns
dermatophytes
Griseofulvin toxicity
teratogenic carcinogenic confusion HA activate P450 --> inc. warfarin metabolism
