heparin

A

pharm class: Anticoagulant
MOA: inactivates the clotting factors so that it can’t bind to clotting factor Xa
monitor with aPTT = 2x normal
60-80 is normal on heparin
give IV or SubQ
warning if liver or kidney disease causes increased bleeding
preferred anticoag in pregnancy
ANTIDOTE: Protamine Sulfate

pharm class: LMWH
inactivates clotting factor xa but not thrombin
prevent DVT
bleeding is less

warfarin (Coumadin)

A

pharm class: oral anticoagulant
MOA: blocks the synthesis of clotting factors
takes several days to see effect
monitor with PT and INR : anticoag levels is 2.5-3.5
pregnancy X don’t use and is excreted in breast milk
has the most interactions with drugs
ANTIDOTE: vitamin K or fresh frozen plasma
food interactions: vitamin K

reverses effects of warfarin over 36 hrs
vitamin K is given to newborns
