if a patient is bleeding or at risk of bleeding (including due to a
recent ischaemic stroke) they should not be prescribed warfarin or
heparin

A patient with peripheral arterial disease (usually indicated by absent foot pulses) should not be prescribed compression
stockings (which may cause acute limb ischaemia)
