what age does it most commonly occur in?
what are the risk factors?
A
- men more than boys
- 11% of GUM clinic attendances
- diabetes
- poor hygiene in uncircumcised males
- immunosuppression
- chemical irritation

what kind of infective organisms does it tend to attract?
A
- candida (esp if immunocompromised)
- staph/ Group B Strep
- STIs: trichomonas, treponema pallidum, HSV, HPV,

what dermatological causes are there?
A
- lichen planus
- leukoplakia
- seborrheic dermatitis
- pemphigus/ pemphigoid
- Bowen’s disease (penile)
- circinate balanitis (reactive arthritis)
- drug reaction: sulphonamides and tetracycline

how might the patient present?
A
- swollen glans
- phimosis
- penile ulceration/ plaques
- purulent/ foul smelling discharge
- impotence/ pain during sex

what investigations would you like?
A
- HbA1c
- send swab for MC+S
- if STI send to GUM clinic

how should they be managed?
A
- daily clean with warm water (saline baths)
- if an STI suspected then screen partners
- manage diabetes

how should it be treated if dermatitis is the cause?
candida?
bacterial infection?
A
- avoid triggers, treat cause
- fluconazole
- ## send to GUM (flucloxacillin/ erythromycin)
