cachexia
body wasting d/t malnutrition
cachexia causes
CINV, mucositis, ulcers, thrush, anorexia, altered taste
external radiation
pt is NOT radioactive, no danger to others
internal radiation
pt is radioactive, risk to others until removed
rad side effects
radiation dermatitis, risk of secondary malignancies, systemic side effects
radiation dermatitis
redness, rash, desquamation- dry, itchy, prone to breakdown
skin pro w rad
wash qday w/ water & gentle soap, rinse thoroughly, use hands not washcloth, use prescribed products, avoid sun exposure
systemic SE of rad
altered taste, fatigue, bone marrow suppression
nadir
point when bone marrow activity & WBC are lowest - highest risk of infection
chemo doses
based off height & weight - update often since weight loss is side effect
chemo admin through central
check for blood return - high extravasation risk
chemo admin safety
PPE, double sign off
chemo admin PPE
mask, face shield/goggles, gown, chemo gloves or double glove - at least 48 hours after
oral chemo considerations
just as toxic - PPE
DNC, split, break, or chew
bone marrow suppression causes
pancytopenia
filgrastim, peg filgrastim
monitor WBC & ANC, can cause bone pain
Epogen alfa, darbepoetin
monitor RBC

bone pain, headache, n/v, dizziness, fatigue, swelling
CINV meds
dexamethasone, lorazepam, metoclopramide, aprepitant, ondansetron
mucositis care
thorough oral assessment, clean & well fitting dentures, no mouthwash w alc - rinse w baking soda or salt water, no tobacco/alc
what foods should mucositis pts avoid
spicy, salty, acidic, dry, hard/crunchy foods
nothing too hot
alopecia pt teaching
it will grow back - may look different; keep protected in summer, wear hat to keep warm in winter
chemo brain s/s
reduced concentration, difficulty learning new info, memory loss, confusion
chemo brain tx
stay physically active, read books, take meds
