- increased oestrogen
- poorly controlled diabetes
- immunosuppression
- broad spec abx

Presentation of vaginal thrush
A
- thick, white, ‘cottage cheese like’ discharge without odour
- vulval + vaginal itching
- vulval soreness
- dysuria
- dyspareunia

Investigations of vaginal thrush
A
- often treatment is started on presentation + Ix not needed
- vaginal pH to differentiate between BV, candidiasis + trichomonas
- charcoal swab + microscopy

Management of vaginal thrush
A
- first line: single dose oral fluconazole
- single dose clotriazmole cream or pressary at night
- or three lower dose pessary
- Canesten duo: OCP - single fluconazole tablet + clotrimazole cream
- alternative contraception needed for 5 days after cream or pessary - can damage latex condoms + prevent spermicdes from working

What advice should be given regarding thrush treatment and contraceptives

- Pessaries + creams can cause damage to latex condoms + prevent spermicides from working
- Alternative contraception needed for 5 days after use

A
4 or more episodes a year

Diagnosis of candidiasis
A
High vaginal swab for microscopy + culture

Management of recurrent vaginal thrush
A
- induction-maintenance regime
- induction: oral fluconazole every 3 days for 3 doses
- maintenance: oral fluconazole weekly for 6 months
