Cardiology Flashcards
Diagnosis (24 cards)
Young athlete , with sudden onset cardiac symptoms
hypertrophic obstructive cardiomyopathy
Common cause of death in HOC
Ventricular arrhythmia
persistent ST elevation on ecg and no chest pain following recent MI
Left ventricular aneurysm
Poor prognosis in hypertrophic cardiomyopathy
reduced left ventricular outflow gradient
Features of HOCM
young man with exertion dyspnea, echo: mitral valve regurgitation, asymmetric hypertrophy , systolic anterior motion of anterior mitral valve leaflet
Features of right heart failure
ankle swelling , dyspnea , fatigue , hepatomegaly, JVP elevated , pan systolic murmur heard best at the left sternal border
ECG findings in Right heart failure
Right axis deviation, exaggerated v waves
Brugada syndrome
convex st elevation in v1-v3 , negative t waves and partial right bundle branch block
Brugada syndrome
caused by mutation in myocardial sodium ion channel protein
Causes of transudative pleural effusion
CHF, Cirrhosis, Nephrotic syndrome, pericarditis, pulmonary embolism
Causes of exudative pleural effusion
Infection, malignancy, connective tissue disorder, abdominal disorder e.g pancreatitis, chylothorax
Best for investigating stable angina
CT coronary angiogram with contrast
Mechanical valves , target INR
Mitral - 3.5 aortic -3.0
Clinical features of aortic stenosis
ejection systolic murmur, carotid brut, dyspnea on exertion.
Features of complete heart block
syncope, wide pulse pressure, JVP- cannon waves , variable intensity S1, bradycardia.
Epstein’s anomaly
tricuspid regurgitation - holosystolic murmur - worse on inspiration
To reverse bleeding with Dabigatran
Idarucizumab
Atrial septal defect
Risk increased in children with Down syndrome
ejection systolic murmur worse on inspiration and fixed splitting of second heart sound on auscultation.
Causes of drug induced lower lung fibrosis
Amiodarone
Congenital LQT syndrome with deafness
Jervell and Lange Neilson syndrome (Autosomal recessive)
Congenital LQT syndrome with no deafness
Romano-ward syndrome
Arrhythmogenic right ventricular cardiomyopathy
T wave inversion in v1-v3 , epsilon wave form - notch at end of qrs complex
Tetralogy of fallout
Pulmonary stenosis
right ventricular outflow tract obstruction
right ventricular hypertrophy
overriding aorta
right to left shunt
Aortic stenosis causes
< 65 years - bicuspid aortic valve
> 65 years - calcification
