Angina drugs algorithm
- BB (e.g. atenolol…)
- rate-lim CCB
- dyhydro CCB + BB
- add/mono of… ivabradine, nicorandil, ranolazine
- GTN for acute attacks
Heart failure drugs algorithm
- ACEi + CSBB (atenolol…)
- add spironalactone
- add ivabradine if high heart rate
- add salcubitril valsartan if poorly controlled symptoms
- add digoxin if worsening/severe
When is rate limiting +/- dihydropyridine CCB’s used?
Rate limiting - angina 2nd line, A fib rate contol
Dihydropyridine - combo with BB (angina), hypertension
When do you use the different diuretics
Loop - fluid overload e.g. oedema, heart failure
Thiazide like - hypertension
What are drugs used for pulmonary oedema
ACEi, nitrates, furosemide
What are anti-platelets and their indication
Aspirin and clopidogrel - prevent arterial thrombosis
What are anticoagulants and their indication
LWMH, warfarin - venous thrombus (DVT), PE, A fib
What are the timeframes and indications for STEMI and NSTEMI
- STEMI within 120 - PCI, thrombolysis if PCI not available
- STEMI outwith 120/NSTEMI - anticoagulate, nitrate, assess for angiography, fibrinolytics
When are BB used
angina, heart failure, symptomatic myopathies, a.fib rate control, long QT/v tach on demission, recurrent arrhythmias
When is adenosine used
After valvasa massage in SVT

Magnesium sulphate
When is amiodarone/DCCV used
Haemodynamic instability/unstable
What are positive ionotropic drugs
Increase CO e.g. digoxin/amiodarone
What are negative ionotropic drugs
Decrease cardiac workload e.g. BB/rate limiting CCB
What is digoxin used for and facts?
slows HR and improves ventricular filling - heart failure, rate control in A.fib
Has a narrow therapeutic window
