What are ways we can improve the dispensing process, what are tools that might help us reduce our risk to patients?

- Near miss logs
- Incident management and recordings

What is a near miss ?
A
An error that is made but identified and corrected before final sign off supply to the patient

What do you do on near miss logs?
A
- Record details on “near miss logs”
- Review log regularly, looking for particular trends but also consider why each near miss happened
- Put into place any actions that will help prevent similar errors happening in the future

Examples of an “incident”
A
- Dispensing incident: A mistake that has been signed off as patient ready (e.g wrong drug, form, strength, quantity ect)
- Failure to supply meds
- Incorrect advice given when selling OTC meds
- Controlled Drug discrepancies
- Adverse reactions involving a pharmacy service

What is the aim of standard operating procedures?
A
Rules of procedure that should be followed, which is signed by the RP once fully implemented in the pharmacy which should read by all staff!
