What is a near miss ?

A

An error that is made but identified and corrected before final sign off supply to the patient

What do you do on near miss logs?

A
  • Record details on “near miss logs”
  • Review log regularly, looking for particular trends but also consider why each near miss happened
  • Put into place any actions that will help prevent similar errors happening in the future

Examples of an “incident”

A
  • Dispensing incident: A mistake that has been signed off as patient ready (e.g wrong drug, form, strength, quantity ect)
  • Failure to supply meds
  • Incorrect advice given when selling OTC meds
  • Controlled Drug discrepancies
  • Adverse reactions involving a pharmacy service

A

Rules of procedure that should be followed, which is signed by the RP once fully implemented in the pharmacy which should read by all staff!
