What is the first priority in a Code TBI?
ABCs – Airway, Breathing, Circulation.
What neurological scale is used for baseline assessment in TBI?
Glasgow Coma Scale (GCS).
What GCS score may indicate the need for intubation?
GCS ≤ 8.
What should you assume until proven otherwise in TBI patients?
Cervical spine injury – maintain C-collar and spinal precautions.
What are signs of increased intracranial pressure (ICP)?
Vomiting, bradycardia, hypertension, unequal pupils, altered LOC.
What vital sign goals should be maintained in TBI?
SpO2 > 94%, PaCO2 ~35 mmHg.
What imaging is urgently needed for suspected TBI?
STAT non-contrast head CT.
What IV access should be established during Code TBI?
Two large-bore IVs (18G or larger).
What labs should be drawn in Code TBI?
CBC, BMP, PT/INR, PTT, glucose, tox screen, type & screen.
What medications are used to manage cerebral edema in TBI?
Mannitol or hypertonic saline.
What is Cushing’s triad a sign of?
Severe increased ICP – includes bradycardia, widened pulse pressure, and irregular respirations.
How should the patient be positioned to reduce ICP?
HOB elevated to 30 degrees, with neck in neutral position.
What should be documented frequently during a Code TBI?
Neuro checks, GCS scores, interventions, and changes in status.
